How do I access the new patient portal?

If you used the previous patient portal to access your images and reports you will need to create a new account. Please call the performing location and ask them to send you an email to complete this process. If you have already created an account you can click the link below to access the new patient portal.

SDI has all of the latest medical technology including CT scan, ultrasound, MRI, myelogram and arthrogram equipment as well as certified medical personnel who oversee all procedures.
