MyHealtheLife is a patient portal that provides you with secure, online access to your personal health information at Stony Brook Medicine powered by CernerHealth. The portal permits you to review details of your own health records and medical history, as well as exchange health-related messages with your doctor.

If you are having difficulties please call patient relations at (909) 883-8611 to have them update your account. It is your responsibility to keep your PIN number private. What is a PCP?

Your primary care physician (PCP) will oversee all of your health care, including preventive care, physical exams and referrals to specialists. Family practice and internal medicine physicians are available as PCPs.

How long does it take to change your PCP?

It usually takes two weeks from the time you choose a PCP for this change to be effective with your insurance. To schedule your first appointment, call: Family practice/internal medicine at 1-909-883-8611, ext. 5030. Internal medicine/pulmonary (lung) at 1-909-883-8611, ext. 5000.
