Where does my health information in the Portal come from?#N#All of the information in the Portal comes from your Porter Medical Center Electronic Health Record. This ensures that you have access to the most accurate, up-to-date information possible.

Access the Patient Portal by visiting or calling any health center location for assistance.#N#Patients will need to: 1 Provide a personal email address. 2 Visit UGLHealth.org and click on the ‘My Patient Portal’ button in the top right-hand corner. 3 A new browser window or tab will open with a login screen. The login area is on the right side of the screen. 4 Log in using the assigned username and temporary password. 5 Review and agree to the Terms & Conditions. 6 When prompted, change the temporary password. 7 Full access to the Patient Portal should now be available.

Providers will update results and answer questions during their regular daily visit. If patients have questions about information in the portal, they can ask a member of the care. If a patient receives results after the provider has already done daily rounds, they will be discussed on the next visit.

Yes. Using the portal locator doesn’t affect your ability to use other URLs to log in to your patient portal.
When you submit the Parent/Guardian Access form to gain access to a patient’s UH Personal Health Record (PHR), you will receive an email invitation from FollowMyHealth™. Follow the instructions in the email to accept the invitation and set up access to view the patient’s PHR.
If you are not able to successfully sign up online and you do not have any upcoming appointments, please visit your provider’s office, show a picture ID, and ask to fill out the PHR Paper Enrollment Form.

The UH Personal Health Record (PHR) is a safe and secure way to access your medical records and office visit notes online, communicate with your physician, request appointments and prescription refills, pay your bill and much more .
Parent/Guardian Access is automatically turned off when a child turns 18. Parent/Guardian Access has been removed. In certain cases, the law allows minors to access medical care without their parent or legal guardian’s consent. In these cases, minors may decide not to let their parent or legal guardian see their health information.
