The Stamford Health and Stamford Health Medical Group Patient Portals help engage patients and families in their care by providing a secure and confidential web-based tool for accessing your health information. Please note you may have multiple portals depending on where you were treated and their features may vary.
Enroll into the Stamford Virtual Health Kiosk Patient Portal. In order to enroll, you'll need your First Name, Last Name, Date of Birth and Pin. Your pin will be provided to you at your visit. Once you have entered your information on the home screen, you can establish a username and password, and complete your contact information.
To request access to the My Sanford Chart record of an adult whose medical care you help manage, please complete the Adult Proxy Form and mail to the address listed on the form for processing. In order to process your request, the patient must sign the Adult Proxy Form and provide authorization for release of medical information in My Sanford Chart on the page titled "Adult Proxy Authorization for Release of Medical Information". Please note that the patient's chart will be accessed through your (the proxy's) My Sanford Chart record. Completing this form will establish a My Sanford Chart record for you and for the patient.

The Cures Act ensures patients have access to their electronic medical records via the patient portal without delay. All completed provider notes, laboratory tests and diagnostic tests will be available on the portal for both historical and recent visits, this may also include previously unavailable sensitive results such as HIV, STD, genetic testing, etc. Please make sure only appropriate individuals have access to your portal through the proxy access. Patients who are at the end-of-life (regardless of COVID-19 status) will be allowed to have visitors in small, controlled groups which will be confined to the patient’s room or other appropriate, controlled location. If PPE is necessary for visit, nursing will assist and ensure visitors are educated and follow PPE precautions.
