The MyMartinsPoint patient portal gives you on-demand access to important health information and services, seven days a week, 24 hours a day. Securely log on from any device and quickly connect with your health care providers or view your health care information.

What is the patient portal and how does it work?

The Patient Portal is your personalized health information site. This tool allows you to see upcoming scheduled appointments, a list of current medications and recent lab results. At your first visit with us, we will setup your username and password during the check-in process.

How do I get information from the previous patient portal?

Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that. How do I use the Barry Pointe family care patient portal?

Once you have completed the enrollment and logged in, you will be taken to the Barry Pointe Family Care Patient Portal home page. From there you will be able to send and receive secure messages to and from our office. Just like your regular email, you will be able to send and receive emails from us through the secure NextGen patient portal

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – ShorePoint Health Port Charlotte and Punta Gorda will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.

Note to previous Portal Users

In order to access your account a few simple steps are needed

You can now self-enroll. Simply click on the Patient Portal Self-Enrollment link at the top of the page and follow the on-screen instructions.

View Your Chart

Request a copy of your Health Record and you will be able to see all of your medical history including lab results from Quest and Labcorp.

Appointments

Request an appointment giving times and dates that will work for your schedule for any non urgent issue

Research Center

The Research Center allows you to look up health-related information. Everything from 1,500 calorie diets and menus to how to treat an ankle sprain to better understanding your test results. This is wonderful resource that we feel is very important for patients to have access to.
