Make an Appointment

At Raleigh Neurology Associates, our primary focus is to provide the highest quality care to our patients and families. There are several ways in which you can make an appointment:

Referrals

In some cases, insurance carriers require a referral from a physician. We work closely with all of our primary care offices that can make a direct referral to Raleigh Neurology on your behalf. Once the referral is received, our team will contact you to schedule the appointment and start the journey.

Request My Records

We are happy to assist you. In order to accept and process your request, we need a signed medical release form.

Help with External Forms

There are times when you may have an external form (examples include DMV, FLMA, etc.) that you need completed by a provider. We have a variety of ways you can provide these to RNA so we can coordinate completing them for you. From the time the form is received at our office, there is a 2-4 week turnaround for completion.

Patient Relations

We are dedicated to providing the highest level of quality care for all our patients. It is very important to us that every patient has a good experience.
