What is a ProMedica authorization form?

Completing a ProMedica Authorization Form will give us the permission we need to release your medical records to you. You can also use this form to release the records to another person or a doctor, if needed.

To get started with MyChart, talk to your doctor. He or she will give you information on how to sign up. We may also give you an activation letter after a hospital stay. This letter will walk you through how to set up a MyChart account.
