Please arrive 15 minutes before your scheduled appointment time. There may be a form or application to fill out in order to enroll in one of our many insurance programs. These forms/applications must be completed before seeing a medical provider.
Bring your co-payment or method of payment. Cash, check, debit cards or credit cards are all acceptable methods of payment and some payment is needed for all services rendered.
A medical provider is available to take your urgent call when the Petaluma Health Center is not open, 7 days a week, and will be contacted by our answering service when you call our main phone number, 707.559.7500.
If your billing matters cannot be solved on the phone, you can speak to a representative in person at the Financial Services Office located just down the street from the Main Center: Petaluma Health Center, Financial Services Office.
We have a Patient Assistance Program that helps patients who are unable to afford the full cost of medications. Your doctor can refer you to this program based on need and income level.
