How to get medical records from UHS?

If you have an urgent need for your medical records, please email UHS Health Information Management at UHS-ROIRequest@med.umich.edu during business hours, or fax your request to 734-936-3063.

How to request radiology images at UHS?

Call UHS Radiology directly (734-764-8302) to request radiology images, minimize wait time and ask any questions. Images will be put on a CD to be picked up or mailed.

What is UHS confidentiality?

UHS recognizes and supports patients' rights to confidential health care. Our staff is committed to ensuring your confidentiality, and all staff, both permanent and temporary, sign agreements to that effect.

Why is UHS unable to communicate with parents?

Due to Federal confidentiality law, UHS is unable to communicate with parents without patient consent. This law protects patient privacy and controls how much health information is access and disclosed.

How long does UHS keep records?

Please note that UHS retains records for 10 years past last date of service.

Why is it important to have your health records transferred to UHS?

It is helpful to have your health records transferred to UHS so your health care provider knows exactly what medication or treatment was prescribed.

How old do you have to be to get your health records in Michigan?

If you are 18 years old or older, or an emancipated minor (defined under Michigan law as being married, emancipated through a court order, or on active duty in the armed forces prior to age 18), your health records will be released only with your written consent unless permitted by law.

What to do when UHS is closed?

In case of emergency, call 911 or go to the nearest hospital emergency department. For urgent concerns when UHS is closed, you may wish to: Call for nurse advice, day or night. Go to a local urgent care - see Emergency / After-hours for options (any costs would be your responsibility)

How long after exposure to UHS can you get tested?

You have the option to get tested 5-7 days after exposure through UHS.

Can you request a test for quarantine?

If you are quarantining privately, you can request a test through processes listed above.

Does UHS cover out of pocket costs?

For currently enrolled students and UHS Prepaid members, you will have no out-of-pocket costs. Your insurance will be billed for COVID-19 testing and any remaining balance will be covered by UHS.
