New Patients

Once you complete your online registration, you will be a assigned to a dental student and faculty team who will contact you by video chat to discuss your medical and dental history in further detail prior to your in-person visit.

Need Help?

Please contact us if you need help logging in or navigating the patient portal. If you are unable to access the site prior to your appointment, our staff can help you complete the process upon arrival, however, please arrive at least 30 minutes early so your appointment is not delayed.

How to protect your LCMC patient portal?

As a LCMC Health Patient Portal user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis and 2) Keeping your login ID and password confidential. It is extremely important that you keep your login name and password completely confidential.

How to enroll in LCMC?

To enroll in LCMC Health Patient Portal, you will use your access code. The access code you receive will give you the opportunity to create a personal login name (called “LCMC Health Patient Portal Username”) and a password. Once this access code has been used or after 60 days of assignment, it will no longer be valid.

Appointments

Appointments for the Health Center are scheduled Monday through Thursday (8am – 5:00pm) and Fridays (8am – 3:00pm). Appointments are scheduled according to the treating physician. New patients must arrive 45 minutes prior to their scheduled appointment to fill out paperwork or they will be rescheduled.

Cancellation Policy

Our goal is to provide quality medical care in a timely manner. To do so, we have implemented an appointment/cancellation policy. The policy enables us to do better utilize available appointments for our patients in need of medical care.

Medical Records

Power of Attorney verification is expected at the first visit. Any medical records or test results requested by another physician’s office may be sent by fax/mail at no charge. Patients requesting medical records/test results will be charged $.60 per page. Payment is expected prior to the release of records.

Insurance

Insurance is billed as a courtesy to the patient. Please direct all billing inquiries and account questions to (702) 777-9930. Patients without insurance are required to pay for services in full at the time of service. A minimum of $125 will be collected at check-in.

Accepted forms of payment for services include cash, debit, personal checks, and credit cards (except American Express).
