The MyHealth Patient Portal is a website that offers personalized and secure online access to portions of your medical record. It enables you to securely use the internet to help manage and receive information about your health. MyHealth only displays patient information that occurred on or after October 30, 2016.

What is MyHealth Patient Portal? MyHealth Patient Portal provide secure access to an online view of your health record, showing one single record of information, regardless of whether a patient was seen at Hamad Medical Corporation (HMC) or Primary Health Care Corporation (PHCC).
My Penn State Health is a secure website where you can conveniently manage your health care information related to your visits with Penn State Health providers.
You can create your account online using the Self-Enroll process. Or you may sign up with the office staff during any visit to the Milton S. Hershey Medical Center or one of our Medical Group locations.
Do not use My Health in the event of an emergency. For medical emergencies, please call 9-1-1 or your physician's office immediately.
NOTE: Self-enrollment is only available for patients 14 years or older.
Yes, messages sent and received via My Health automatically become part of your medical record.

Simply contact your physician’s office and speak to a representative about enrolling in the myCKHealth Patient Portal. The office staff will then provide you with the myCKHealth Patient Portal Welcome Handout. This handout will include step-by-step directions and a Patient Identification Number (PIN) needed to set up your account.
