Patient Portals Temple Health’s patient portals provide users with the ability to access portions of their electronic health record and manage many of their day-to-day healthcare tasks. These portals are available to anyone who has received care at one of our three hospitals or from a Temple or Fox Chase physician.

Problems Logging into Patient Health Portal

If the error message you receive is " We have no record of you in our database.", it means your record is not available in our system yet. New patient uploads are done Monday nights. Please check back on Tuesday to see if your record is available.

Problems Uploading Documents

Simplify the file name by renaming the file (i.e. MyNameDoc.pdf). Make sure the name of the file is less than 30 characters long.

Request An Activation Code Online

You can register online by visiting my.TempleHealth.org, clicking on the new user sign-up and completing the activation code request form. Follow these steps:
