How do I obtain an authorization to seek care at Summit Pacific?

If you are new to the organization and would like to seek care at Summit Pacific, it is required for you to call the Veterans Affairs office to obtain an authorization. Please have this authorization in hand along with the valid dates of service when you call to schedule your new appointment.

What is a patient centered medical home?

This means our patient care teams use a team approach that focuses on the patient’s health, and the overall health of the patient’s family unit. This approach helps encourage two-way communication and participation in care plans between the patient and their provider; it also encourages patients to take a proactive approach to managing their own health and the health of their family. The Medical Home project allows more one-on-one time between provider and patient and enables the patients to participate in their healthcare decisions.

Family medicine residents are physicians in training who work alongside board-certified family medicine physicians and inter-professional health care providers to promote continuity of care for family medicine patients.
