This approach helps encourage two-way communication and participation in care plans between the patient and their provider; it also encourages patients to take a proactive approach to managing their own health and the health of their family. Family medicine residents are physicians in training who work alongside board-certified family medicine physicians and inter-professional health care providers to promote continuity of care for family medicine patients.

Property Tax Credit: Patients living within the district may receive a credit in the amount of the public hospital district portion of their property taxes assessed within the same year of the related date of service, up to a maximum annual credit of $500.
