What is our patient portal?

Summit Medical Group's new patient portal allows patients to request and view appointments, send messages or refill requests, and view test results and billing online without having to schedule an appointment or wait on the phone.

What is Summit Community Care?

Summit Community Care Clinic is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services through a team-based model of care. It is the only clinic in its 5-county service area that offers a comprehensive sliding fee scale, primary care to Medicaid and low-income, uninsured patients, and is the only safety net dental provider.

Does Summit Community Care receive HHS funding?

Summit Community Care Clinic receives Health and Human Services (HHS) funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

I f you would like to assign a family member or someone you have a close relationship with, access (proxy) to your My SummitCare account, complete the My SummitCare Proxy Access Request Form below and return it to the medical records kiosk on the first floor of Summit Pacific’s Wellness Center.

Will my medical record from the previous patient portal transfer over to Follow My Health?

No, when you register for the new Follow My Health Portal please be aware that none of your medical history from the previous health portal will be transferred over to the Follow My Health Portal. Rest assured your previous medical visits and your medical records from the former portal are still available to you and your healthcare provider.
