1. Please complete the digital intake form emailed to you prior to your visit. It is the preferred, comprehensive intake method which will ensure a more productive visit with us. If you are unable to complete the digital form, please download and complete the appropriate intake form below.
Initial appointments are typically between 60-90 minutes long. During this visit, you and your doctor will discuss your health concerns and you will be given the opportunity to share you medical history. Your doctor will perform a physical exam and, if necessary, discuss tests that would be useful in clarifying your condition and needs.
Return visits are typically 30 minutes in length. These visits are designed as check-ups to see how you’re doing, note positive changes, and make adjustments to your personalized treatment plan. To best suite your needs, a physical exam maybe performed at this visit.
We ask that you give us at least 24 hours notice if you need to cancel or reschedule your appointment. If you cancel your appointment without 24 hours notice or miss your appointment, you will be charged a $50 fee.
