How do I sign up for the clinic portal?

To get registered for the clinic portal, please call the clinic at 541-966-0535 or ask for access at your next appointment. Once you are set up, click the button below to complete your registration and access the portal.

How do I Reset my Password in the patient portal?

Sign in to the Patient Portal, go to Security Settings, and reset your password. Click Forgot your password on the sign-in page and enter your email address to request a password reset email. Contact your provider's office and request a password reset email. + What if I forgot my password?

Who is eligible to register for the patient portal?

Any active patient may be eligible to register for and use the Patient Portal. If you are authorized, a family access account can be created that will allow you to access selected family members' health information.

Hospital Patient Portal

This portal houses information from all services performed at the hospital, including laboratory testing done at St. Anthony Hospital, Diagnostic Imaging reports, Visit History, Health Summary, and Discharge instructions. You may be issued a code to log on to the portal during your visit to St. Anthony Hospital.

Clinic Patient Portal

This portal houses information from all services through the St. Anthony Hospital physician clinics, including the family clinic, walk-in clinic, women's clinic, urology, and orthopedic clinic. Here you will find information your Visit History, Health Summary, and Discharge instructions.
