Patient Portal Neighborhood’s Patient Portal is a secure and personal way to manage your health care online. Through the Patient Portal, you can review doctor’s notes, get your lab results, update your personal information, request refills for your prescriptions, send and receive messages from your Care Team, and schedule an appointment.

Your care team’s primary responsibility is to work for you. They continually review your care and progress and coordinate your care under the leadership and expertise of your Primary Care Provider. Your care team’s goal is to help you improve your overall health.

What is Reach Out and Read?

Reach Out and Read is a national literacy program that distributes new books to infants and children up to the age of five. Children receive age-appropriate books at their wellness exams. Through the Reach Out and Read program at participating Neighborhood locations, more than 9,000 children receive a new book to take home and keep every year. Parents learn the importance of reading to their children out loud, which increases vocabulary and language skills, and ensures children are better prepared to enter preschool and kindergarten. Reading to a child also builds curiosity, cognitive ability, and strong memory skills, even before a child learns to talk. Neighborhood staff members work with families during pediatric exams to model reading out loud with their children. What is a complex care resource specialist?

Our Complex Care Resource Specialists are here to help you with housing, food, transportation, employment, benefits, and other social service issues. At Neighborhood, we understand that gaps in these basic needs can account for at least 40% of health-related outcomes. We’re committed to improving the health and happiness of everyone in our communities. We have built strong partnerships with community organizations that specialize in providing social services designed to help you and your family achieve stability and thrive. At Neighborhood, we assess all individuals for social service needs and connect them with the resources they need to be healthy and happy. For more information on social service assessment, coordination, and referrals, contact your local Neighborhood location and ask to speak to a Complex Care Resource Specialist.

Why are teens called invisible patients?

Teens are often known as the “invisible patient” because they don’t go to the doctor for regular appointments between the ages of 12 and 20, missing out on needed immunizations and health maintenance. Classes: Teens receive health-related education on topics that are relevant to their day-to-day lives.

What is Medi-Cal for?

Medi-Cal is a program that offers free or low-cost health coverage for low-income adults, families with children, seniors, persons with disabilities, pregnant women, children in foster care and former foster youth up to age 26. Medi-Cal’s enrollment is year-round.

What is Calfresh program?

CalFresh (also known as SNAP, Food Stamps, or EPT) is a program for low-income families and individuals that meet certain income guidelines. CalFresh benefits help supplement your food budget and allow you and your family to buy nutritious food.

What is feeding America?

Feeding America. Feeding America is the leading hunger-relief charity in the nation. Through both local and national food assistance programs, Feeding America is able to provide nutritious foods to families struggling with hunger.

What is CPSP in meds?

Our Comprehensive Perinatal Services Program (CPSP) offers a wide range of services to pregnant women on Medi-Cal, from conception through 60 days postpartum. In addition to standard obstetric services, eligible women receive enhanced services in the areas of nutrition, psychosocial, and health education. What is continuing nursing care?

Continuing Nursing Care is for patients who require continual nursing care. Patients in this area receive 24-hour nursing care and supervision for support in their daily activities, which includes help with bathing, dressing, meals, incontinence care, mobility and managing medication. These patients, through our care, are able to achieve and maintain the highest possible level of function.

What is memory care?

Our memory care centers provide a comfortable and protected world where patients with moderate memory impairment can live a lifestyle that provides a genuine sense of home and belonging. Patients have a beautiful, secure and caring environment free from daily burdens. Our centers are fully secure, with highly trained medical staff, emergency response systems and personal assistance with medication and personal care.

What is skilled nursing?

Skilled nursing provides a bridge between home and hospital, with 24-hour nursing care provided by licensed nursing teams. Whether provided for several weeks or several months, our physician directed skilled health care services are designed to complement a hospital stay.

What is assisted living?

Assisted living and residential living provide thoughtfully designed private living spaces, inviting community areas and daily activity schedules that contribute to the feeling of home. We create personalized plans for each resident’s health needs and preferences. Licensed medical supervision provided on-site 24/7 adds to a feeling of safety and security.

What is independent living?

INDEPENDENT LIVING. Independent living means enjoying a new way of living. This is the perfect option for those individuals, 62 years and older, who wish to maintain active, independent lifestyles without any home maintenance worries.

Pursuant to the 21st Century Cures Act, Core Physicians, LLC provides patients and their designated representatives with essential clinical data, including core data classes and elements in the U.S. Core Data for Interoperability (USCDI).

Southern New Hampshire Health has transitioned from offering the eCare patient portals to SolutionHealth MyChart.
