Obtaining a Copy of Your Medical Records

Access to medical records is available to patients over the age of 18 or by a legal guardian.

Records Are Available in Two Ways

Most of your records, including physician notes, labs and other tests are available through your My National Jewish Health patient portal account.

Request for Amendment of Medical Records

If you would like to make a request to amend your medical records, please complete the Request for Amendment of Health Information Form and return it to the Health Information Management department at National Jewish Health.
