Advance Care Planning

It’s easy to be overwhelmed by what we are experiencing as we create a new normal during the COVID-19 pandemic. There’s a lot that you can’t control, but one thing you can control is what health care decisions are made in the event of an emergency.

Ensure you have a health care directive

A health care directive is a written document that informs others of your health care wishes. It allows you to name a person ("agent") to decide for you if you are unable to make a decision. You must be at least 18 years old to make a health care directive. To get started, download a health care directive from Honoring Choices Minnesota.
