How do I submit my medical records to IUPUI?

Submit health records: 317-278-6929 (Fax) or healthsv@iupui.edu. Please be sure to include your name and date of birth on the first page of your documents. If you’d like to make an appointment for counseling or mental health services, contact Counseling and Psychological Services.

What is the patient portal and how does it work?

The Patient Portal is a great way to ask questions about your health care or request prescription refills as well as access your medical records, lab test results, and immunization history. If you have a particular provider—a doctor or nurse practitioner—you’d like to see, you can request them when you schedule your appointment.

How do I request an appointment with an IU Health Doctor?

If you are not yet an IU Health patient, please request your first appointment or call 1.888.IUHEALTH to request an appointment.

Are the clinics at IUPUI open for TB testing?

Our clinics are completely booked up. If you are still needing vaccines or TB testing you can set up an appointment at IUPUI Campus Health at Coleman Hall. Please call 317-274-8214 to schedule that appointment.

Quarantine and isolation resources

The testing, quarantine, and isolation processes can be overwhelming, so we’ve prepared guides to help you know what to do and what resources are available through each process. The information is also specific to whether you’re living in on-campus housing or living off campus.

General medicine

We can handle most of your medical concerns, including sinus infections, ear aches, colds, flu, pink eye, abdominal pain, and more. And when necessary, we can write the prescriptions you need to get well or manage your condition.

Sexual health

Get the sexual health answers you need in a judgment-free environment. We have many years of experience helping patients with a variety of sexual health concerns, symptoms, and questions. Sexual health is important across all sexual orientations and gender identities. We are here to help ALL students.

Immunizations and travel medicine

One of the most important things you can do to safeguard your health is stay current on your vaccinations. You can get many of the immunizations you need right here, including:

All health care and beginning international students must get a TB test.
