Could you be a new patient at Firelands?

We are accepting new patients. Find your primary care provider and learn about the new facility, opening this October. You could be part of the Firelands family. Work alongside other caring team members and receive competitive benefits.

What reports will be posted in the patient portal?

Reports such as History and Physicals, Discharge Summaries, Operative Reports, Physician Notes and more will post directly into your patient portal upon finalization by the physician’s signature. Pathology reports will be following shortly after, in June.

What can I do with the patient portal?

With the patient portal, you can view select test results, access your medical records and track your health information. To enroll in the patient portal, you must receive services at one of our locations.

How do I enroll in the patient portal?

To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.

For your convenience, our central billing office is staffed Monday through Friday from 8 a.m. to 4:30 p.m. You can call the office at 419-557-5530.

Advance Directives

Advance directives are legal written documents that indicate what an individual wants regarding care and treatment if he or she is not able to speak for himself or herself. There are three documents: a healthcare power of attorney, a living will and a do-not-resuscitate order. All must be signed by the competent patient and witnessed or notarized.

Can an adult patient grant another adult access to their patient portal?

An adult patient may grant another adult access to their patient portal. Legal Guardian Portal Proxy Authorization Form. A Guardian or a Person with an Active Durable Power of Attorney for Healthcare or another valid legal document may apply to become a proxy for an adult patient.

Do you need a proxy form for a caregiver?

If you are a caregiver and wish to gain access to your children (s), spouse, parent, or loved one's medical records, a proxy form needs to be filled out and signed by the patient as well as the caregiver. Below are the appropriate forms and corresponding explanations.

At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.
