If you have an Apple or Android mobile devices, you can access the Emory BLUE and GOLD Patient Portals through a free app called HealtheLife. Once you’ve donwloaded the app, log in with your username/password for either the Emory BLUE or GOLD portal.

All Rollins MPH and MSPH students are required to complete a 200-400 hour applied practice experience. This requirement is designed to enhance the student’s understanding and application of knowledge and research findings to public health settings by providing an opportunity to gain practical experience. Public health work environments include nonprofit organizations, hospitals, local health departments, and for-profit firms.
The Rollins School of Public Health will accept these test scores for students who wish to submit them. Should students indicate they are submitting GRE scores on their SOPHAS application, Rollins will hold the application for admission until scores are received.
Preferably, you can contact us directly via your Patient Portal by sending a message to “immunization compliance.” Please keep in mind that your immunizations are due by July 1 st for new to campus UCOL students and August 1 st for returning students and new graduate and professional students.
Fill out the online Tuberculosis Questionnaire Form on the Your Patient Portal home page. You may also fill out the paper version of the Tuberculosis Questionnaire on the paper immunization form and upload with your signed immunization records. Note: an alternate tuberculosis screening is required for the School of Medicine, all Allied Health programs, and all School of Nursing programs.
If you received an exemption last year due to being entirely remote, that has expired and you will need to upload your immunization records prior to August 1st. If you are living on campus, a Meningococcal ACWY vaccine is required.
Emory University Student Health Services (EUSHS) provides outpatient care for enrolled Emory students with a valid Emory ID card. International student's spouses, Domestic Partners and unmarried children over 18 years of age are also eligible for primary medical care if they are currently enrolled in the Emory/Aetna Student Health Insurance Plan.
To request a declination, go to the Student Portal and find ‘Forms’ on the left side, then ‘Immunization Forms”. Scroll down to the bottom of the page and click on the link to the declination form. Upload the completed form. For a medical contraindication we need a letter from your provider explaining the contraindication. The letter needs to be signed by your provider and yourself. Upload the form through the student portal.
If you or your healthcare provider feels that you cannot comply with any of Emory’s immunization requirements (including medical contraindications to specific vaccinations), please submit a letter of explanation signed by both you and your healthcare provider along with your updated immunization records using the Student Patient Portal to upload.
A copy of the Varicella titer lab report must be submitted/uploaded if that option is chosen.
