How to contact CFM?

For appointments within a few days please stop by or call us at 541-757-2400. For CFM patients with urgent medical needs, on-call doctors are available 24 hours a day and same day appointments are available. CFM doctors’ hours vary, so please ask the receptionist about your doctor’s schedule.

Can I make an appointment at CFM?

An appointment is required to see a doctor at CFM. If you are already enrolled as a patient at CFM you can schedule an appointment using the Patient Portal or Healow mobile app. This is to schedule appointments more than a week out.
