On any medical record you may have. On a recent billing statement (see image below) If you need your Medical Record Number, call 535-7711 and ask for Health Information Management.
Most often, your user name is your email address that the CMM team has on file.
Once you have entered this information, a message will be sent to your email address providing you with a link to reset your password.
The CMM Clinical Center’s FollowMyHealth® Patient Portal is an internet-accessible application that offers patients secure access to pieces of their Colorado Mountain Medical electronic medical record.
After you request a connection to the Colorado Mountain Medical please allow one to two business days for your connection to be reviewed and established. If you experience any problems please contact our Patient Portal Support team. This team is available Monday through Friday 6am-6pm MST (excluding federal holidays) by phone (888) 670-9775.

Patients will be able to access pieces of your electronic medical record, which includes discharge summaries, discharge instructions, outpatient first registration reports, inpatient history and physical examination notes, rehabilitation notes, procedure notes and some laboratory, cardiology, and neurology results.

As previously noted, on July 1st, 2017 Conway Medical Center and Conway Physicians Group began using a new Patient Portal. The records stored on the previous Patient Portal did not transfer to the new Patient Portal. But do not worry, you can still obtain your records from the CMC Medical Records Department or from your Conway Physicians Group providers office.

Patients can make updates to health issues list to notify clinic staff of changes. Medications - including patient's current medications along with pertinent information for each medication, such as the prescribed dosage and the name of the clinician who placed the initial order.
PatientLink is CAMC’s patient portal. A patient portal allows you to electronically access your health records and other information about your care. Once you are enrolled in PatientLink, you can use it from your computer, tablet or phone. PatientLink is convenient, free and secure.
To get access to CAMC PatientLink, you can enroll at registration during your next visit to CAMC. When you give your email address and a secret question and answer to our registration representative, you will receive an email to finish the enrollment process. After you complete those steps, you will be directed to log into the patient portal.
Most password resets can be done by the user within the patient portal by clicking on the "Forgot Password" link on the login screen.

PatientLink is a convenient way to view your medical records and other information about your care. Test results (lab, radiology, pathology) Medication lists. Allergies and immunizations. Summaries of your inpatient and outpatient visits. Health information and education that your health care provider recommends.
If you are a patient of a CAMC-employed physician, you can use PatientLink to send a message to your physician and staff, and request appointments and prescription renewals.
