So that your test date goes smoothly, please remember to: 1 Bring a written physician’s order for the test (this is necessary for all lab work). 2 Bring eyeglasses with you to complete paperwork if necessary. 3 Bring your health insurance card with you so we can create a copy on file. 4 Be prepared to provide a urine sample if your physician has ordered one. 5 Wear loose-fitting or short-sleeved clothing (you may bring a sweater or jacket, as temperatures are cooler in labs).
