You must be 18 years or older to use self-enrollment. Self-enrollment is for patient use only. You will need your medical record number to enroll. You can find this number on your discharge paperwork. Please contact Medical Records at 330-363-6256 if you need to obtain your medical record number. You can enroll by clicking here.

Your invitation will be sent at the time of registration. You will receive a reminder email if you have not set up your account after seven days. The invitation will expire after 90 days.
