Go to MedPortal.AppState.edu. Sign in using your Appalachian State University (AppState) User Name and Password. Select the Immunization Tab --> then select Print. This will allow you to view all Immunizations that you have submitted to AppState and all vaccinations administered by the AppState Immunization Clinic.
