To help protect your privacy and identity, we require patients to present a valid photo ID and supply a current email address in person only.

The first time you login to the Healow App you will need to use the username and password for the Patient Portal. If you do not yet have a username and password or if you have forgotten it, please click your location above and then click Access Patient Portal. To create a new patient portal account, click “Enroll Now”.
