There is a $25 fee for all returned checks. A $25 charge will be assessed to your account if appointments are not kept or rescheduled 24 hours in advance of appointment time. RX Refills

You may send prescription refill requests directly to your provider via the patient portal. Please allow 48 to 72 hours for refill requests. If you have an urgent medication request, please call our office.
