NOTICE: You must have a Patient First Portal account in order to view and/or transmit your lab results and visit information. All first-time Patient First Patients are given a Portal Activation Code during registration.

How do I sign up for the patient portal?

Go to portal.patientfirst.com. Under “First Time Here?” enter your activation code and your date of birth. Follow the easy steps to set up your secure Patient Portal account and create your Username and Password. Under “Returning User?” enter your Username and Password.

How do I retrieve my activation code for the patient portal?
