If you're at least 18-years-old and a Concord Hospital or Concord Hospital Medical Group patient you can request to enroll in My Patient Connect. Once you receive your invitation in your email account, you can register online. Proxy access is the official name for gaining access to another person's account.

Any information you have typed will be lost. We recommend that you do not leave your PatientConnect account open if called away. Always log out.

The patient portal for Concord Hospital - Laconia and Concord Hospital - Franklin is separate from My Patient Connect. In the future, the two portals will be combined but for now, patients need to log in to separate portals based on where they receive their care.
