Who is Partnership HealthPlan of California?

You can fill out a release before your appointment and bring it with you, or you can visit one of PHC's clinics for help filling out your release in person. Please call to schedule an appointment. To register online click here. Once your registration has been completed, we can give you access to our patient portal.

COVID-19 Vaccination Patient Form

If you have scheduled a vaccination appointment with PHC, you will need to fill out the form below. We will have copies available on site, but our process is made much easier if you are able to have this filled out when you arrive.

Other Important Documents

There are two more important informational documents for vaccine recipients. These do not need to be printed and you do not need them to get vaccinated. You will be offered hard copies of these documents at your vaccination appointment, but we encourage you to download electronic versions and decline a hard copy in order to save paper.

What is beacon health?

We have partnered with Beacon Health Strategies (Beacon) to help manage mental health benefits for most PHC Medi-Cal Members with mild to moderate mental health conditions in need of outpatient mental health services.

When will PHC change to Direct Member?

Effective February 1, 2021, with direction from the Department of Health Care Services (DHCS), PHC is changing the "Special Member" name to "Direct Member.". This is a name change only, PHC benefits or how members receive care are not affected.
