You can quickly and easily access things like pharmacy information, forms and documents, publications and resources, departments such as Utilization Management, Disease Management, Quality Improvement, Provider Relations and more. We’re also happy to work with you in person or over the phone to address any questions or needs you might have.

Who do we cover?

Paramount covers several types of members. We cover those that are enrolled in commercial benefits through their employer ( Ohio service area map / Michigan service area map ), as well as Marketplace members in northwest Ohio. We also cover Medicaid members ( service area map) and Medicare members ( service area map ).
