If you are a Paramount subscriber, you attest that, "I have financial responsibility for the minor and adult dependents covered by Paramount. I have obtained the consent of any adult dependents to view their personally identifiable information for purposes of health care benefit management.

How do I Contact Paramount provider relations?

If you are an eligible patient, you will receive a message in your MyChart account to schedule your COVID-19 vaccination. Please note that not all qualified patients will receive the scheduling message on the first day it’s available. Message timing will depend on how quickly ProMedica receives additional vaccines.

Who do we cover?

Paramount covers several types of members. We cover those that are enrolled in commercial benefits through their employer ( Ohio service area map / Michigan service area map ), as well as Marketplace members in northwest Ohio. We also cover Medicaid members ( service area map) and Medicare members ( service area map ).
