If you are insured, we MUST have you bring your insurance card (or at least a photocopy of the front and back of this card) to your first appointment. If you are not insured or have other questions, please click on our billing and financial policies link to understand your options before your appointment.

PROOF OF RESIDENCY

Any documentation that provides proof of your residency within Whitman or Latah County. This can be a bill, contract or piece of mail that includes your name and address.
