Is Pagosa Springs Medical Center secure?

Pagosa Springs Medical Center offers our patients a convenient and secure website and app to help manage your health. HealtheLife connects you with your care providers, lets you check test results, and view personal health information.

Patient Portal

You may sign up for free access to your secure health information through our Health-e patient portal. To sign up, email Informatics.Dept@psmedicalcenter.org. You will receive an email with a link to accept your invitation and set up your account. Once you are signed up, you may access your health information through the Health-e link.

Copies of Your Medical Record

If you are a patient of Pagosa Springs Medical Center (PSMC), you may request access to your medical record by downloading the authorization form below. Please provide the completed form, along with photo identification in person to the Health Information Management (HIM) department, located on the PSMC campus.

Primary Care

The Clinic at PSMC is now closed for lunch from 12PM-1PM each day, due to staffing shortages.

Patient Portal

With the Health-e Patient Portal you get easy, fast, and private access to your medical information and lab results online, so you can view your personal health record whenever and wherever you have access to the Internet. You can now also send and receive follow-up messages with your provider.

Early Detection

Another service provided by our Primary Care facility at Pagosa Springs Medical Center is the Early Detection Program . The program offers $39 cholesterol screens with counseling and follow up, as well as low cost self-ordered lab testing.
