Pediatric Patients

Minors (under age 18) must be accompanied by a parent or legal guardian for lab testing. If you are bringing a minor and you're not that person's legal guardian, you'll need to bring a notarized statement signed by his/her parent or legal guardian authorizing permission for collection and billing of tests ordered by the patient's doctor.
