INSURANCE CONTRACT LIST

We are pleased to participate with most major insurance plans. Below is a list of more common insurances we accept, but this list is not all inclusive. If your plan is not included, please contact your insurance company at the number listed on the back of your card for verification of participation.

Ohio Bureau of Workers' Compensation (BWC)

If you are being seen under a Workers’ Compensation claim, it is important that we have your claim number, date of injury, employer at the time of injury, and all conditions. In addition, we will also need written authorization from your Managed Care Organization (MCO), or your employer, prior to your appointment.

REFERRALS

We advise you to check your insurance plan to see if your insurance carrier requires a referral to see a specialist. If they do, please notify your primary care physician prior to your appointment with our office to obtain that referral.

DURABLE MEDICAL EQUIPMENT "DME"

Medicare and other insurance companies may not pay for all durable medical goods (ace wrap, foot supplies, wheelchair cushions, canes, etc.). It’s a good idea to become familiar with the rules of your health plan if you are injured. Our staff are happy to assist you with understanding what DME needs you might have and what your insurance requires.

HAVING SURGERY?

While our practice contracts with many conventional health care plans including many offered through the government health care exchanges, if you are having surgery, you should also check that the hospital where your physician will perform your procedure is also contracted and in network.

Good Faith Estimate

If you’re uninsured or you pay for health care bills yourself (don’t have your claims submitted to your health plan), Orthopedic One is required to provide you with an estimate of expected charges before you get an item or service.
