One Community Health uses an electronic health record system supported by OCHIN. As a patient, you can enjoy secure, easy online access to your health record and online billpay through OCHIN's MyChart patient portal. You can securely use the Internet to help manage and receive information about your health. Through MyChart, you are able to:
Epic Patient Portal (MyChart) OCHIN Epic EHR also includes MyChart patient portal functionality, which is available in both English and Spanish and provides patients secure access to portions of their EHR online and via mobile devices.
How to link accounts: Step 1 – Log into your Neighborcare Health MyChart account. Step 2 – Click Profile and then click Link My Accounts. Step 3 – After the page loads, scroll down until you see a Search box. Use the Search box where you can look up their other MyChart organizations.
If you enrolled in MyChart after your appointment, your provider will be added to your list of care team members after your next appointment.
Your lab results may not be ready yet, or are still being reviewed by your care team. MyChart will auto-release normal lab results within 72 hours of receiving the results.
Neighborcare Health's electronic health records system helps improve our patients’ experience. Patients have better access to their care and providers through an easy to use online patient portal services called MyChart, provided by OCHIN.
MyChart will automatically remove any adult proxy at midnight of the child’s 12th birthday. There are exceptions to the rule, such as adolescent patients that are developmentally impaired or differently-abled, or an emancipated teen.
MyNeighborcare is no longer active. Please note Neighborcare Health is no longer using our previous patient portal (MyNeighborcare). If you used MyNeighborcare, you will need to sign up for the new MyChart portal to experience all the benefits.
