Please click on the access form button and complete the Request of Information form. This form can be printed and mailed to OrthoArkansas (800 Fair Park Blvd, Little Rock, AR 72204) or faxed to 501-500-3550 . For same day retrieval of records or images, you can bring the form with you to our Midtown location or forms will be provided in the office.

Sign up for access to request appointments, request prescription refills, send non-urgent messages to the office, and update personal and insurance information. To receive an invite to the portal, please ensure we have your correct email address on file. We will then send an invite to your email address.

BEFORE SURGERY

In preparation for your visit, a representative of OrthoArkansas will call you to verify the information we have received from your physician’s office. This process only takes a few minutes and helps to serve you better.

FOR SURGERY

A family member or friend must accompany you to, and stay with you during your surgery. You may not drive after surgery. Public transportation should not be used. Please limit visitors as waiting room space is limited.

AFTER DISCHARGE

Specific instructions prepared by your physician will be sent home with you.
