Our full time chaplain, chaplain resident and trained volunteer Spiritual Care associates provide support with the spiritual and emotional aspects of hospitalization. Spiritual Care is available to patients and families as needed and on request.
If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility, visit MyChart to get an activation code. Once you have your activation code, you can setup your username and password on the MyChart website .
To login to MyChart, visit mychart.baptisthealth.com. Once on the page, enter your username and password that you setup when your created your account and click Sign In.
After logging into your MyChart account, hover over the Messaging tab and select “Ask a Question”
To obtain a copy of your medical records, images or test results, print this form: Patient Medical Records Form. All request forms must be signed and dated by the patient or the patient’s legally authorized representative, parent or guardian. If the patient is under psychiatric care, the supervising psychiatrist will be required to sign the form.
