To schedule an appointment, log into MyChart on a computer, tablet or smartphone. Click on the appointments tab, then schedule an appointment and follow the prompts. Select the time you prefer on your provider’s calendar. No phone call is needed!

How old do you have to be to get MyChart?

Age 12-17 can register for their own MyChart access, and parents will be granted partial access. After a child reaches age 18, parents need to request access. You can request access to a family member's record or give another adult access to your own record by completing a form in MyChart.

Prescription Policies and Information

Your physician will generally prescribe your long term medication in sufficient quantity to cover you until the time of your next appointment. Since many patients obtain their prescription medication from a mail order service, follow up visits should be scheduled early enough to give you time to get your medication renewed prior to running out.
