Digital Forms Submission Instructions (two options)

In order to fill out the form and submit it to OPPT, you must have the latest version of Adobe Acrobat Reader. This can be downloaded for free. You can not submit the digital file via the web browser. The digital file must be downloaded and saved to the computer and opened with Adobe Acrobat Reader in order to be submitted properly.
