One Chart | Patient offers you the opportunity to view your medical history at any time. In One Chart | Patient portal, you will be able to view the medications that you are currently taking and have previously taken. You can also request a refill on your prescription with the click of a button.

Can I grant someone else access to my one chart | patient?

Please note: You can grant someone else access to your own One Chart | Patient account through the Share My Record option without submitting this form, as long as the other person also has their own One Chart | Patient account.
