What happens if you provide complete and accurate information about your insurance?

If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.

What happens if you don't have insurance?

If we do not participate with your insurance carrier or you do not have insurance coverage, payment must be made at time of service unless an acceptable payment plan has been agreed upon prior to the services rendered. If you are not sure we accept your insurance please contact our office, prior to your visit.
