MyChart is Texas Health Resources' secure patient web-portal that allows you to view your medical information, communicate with your Provider, and actively involves you in managing your healthcare. To sign up for your own MyChart account, you will need an activation code.

Currently, NYP is using the Eclipsys Electronic Health Record (EHR) with CPOE. The EHR is interfaced with the pharmacy, nutrition services and the laboratory sytems. NYP has ordersets, clinical alerts and a health maintenance dashboard system.

This is to support the teen’s right to confidentiality since full view access includes clinical information that may be sensitive. On the child's 18th birthday, a parent or legal guardian's proxy access will be terminated.
If you are a parent or guardian of a minor child under the age of 14 and would like to request proxy access to their medical records, you will be required to have your own MyChart account.
The teen can choose to grant limited view access to an adult. If a parent or legal guardian already has full view access to a child's account prior to their 14th birthday, their access will transition to limited view access on that birthday.
Limited does not include medications, diagnoses, clinical notes or instructions, test results. Full access proxies enjoy same features and benefits available to patient, and will see everything patient can see: Clinical notes and test results from all PHS visits, including sensitive health information.

Rest assured, your account remains active and you may continue to access your information (link is external and opens in a new window) . No new health information has been added to your FollowMyHealth account after Feb. 1, 2020, and you can no longer connect to your provider through that account.
The health information in your Connect account comes directly from your doctor's electronic medical record. For outpatient records, you can ask your doctor to correct any inaccurate information at your next visit, or send a message through Connect to your provider’s office to request corrections to your health record.
If you get an activation message via text message or email, you have 24 hours to respond to it. If you get a letter with a code on it, it is good for 30 days. Also, you can complete the Self Sign Up process without an activation code, and the system will ask you questions to verify your identity.
Any patient age 12 or over is eligible for a Connect account. In addition, parents can be given portal proxy access to manage their children's accounts, or patients can grant access to their own accounts to family members, friends, or caregivers. Any adult can be given access to another adult’s account, by granting portal proxy access. The portal proxy does not need to be a patient. The patient will need to complete the Connect Patient Portal Proxy Access Authorization form.
