MyHealthOne Portal: MyHealthOne consolidates many common tasks into one secure, easy-to-use online patient portal. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day. *Note that some medical records may only be available through our hospital Medical Records office.

Paper Copy of Medical Records

Please complete the following steps to obtain a paper copy of your medical records:

For Questions Regarding Obtaining Medical Record Copies, Or To Obtain the Status Of Your Request

Records will be shipped within 5-7 Business days. Should you need to check the status of your request, please call (888) 616-5721 (Option 2)

Urgent Requests, Records for your Physician

For immediate continuity of care, your healthcare provider can request records.

Insurance, Attorney, Disability and Third Party Requests

Requests should be sent from your insurance company, attorney, or Disability Determination Service can be faxed to (833) 462-1884 or mailed to the facility where the patient was treated.

Family Care Specialists in Ocala & Belleview: Mon – Fri: 7:30am – 5:00 pm#N#Urgent Care at Ocala I is also open: Sat 8:00am – 1:00 pm#N#Our offices do not close for lunch.

Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (352) 732-5365 during regular business hours.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.
