You may request a copy of your records on paper, on USB flash drive, or on a compact disc (CD). A completed and signed Authorization to Release Protected Health Information form along with valid identification is required for copies of records to be released. To request the form be faxed or mailed to you, please call 207-662-2211.
Yes. Please be aware that there may be a “reasonable cost-based fee” for copying protected health information in accordance with the State of Maine Statutes. The exact fee amount will not be determined until the request has been fulfilled, at which time an invoice will be sent.
Click on the “My Health” tab to view lab results. Starting Friday, April 16, the test results you see on the FollowMyHealth® patient portal will be posted there as soon as they come in. Faster turnaround of test results means you could see your results in the portal before your doctor sees them and calls you about them.
Sharing your health information with a third-party health care application may lead to further disclosure because it will allow the application access to your personal health information with different policies than MaineGeneral and FollowMyHealth ®.
