Easily access relevant patient information located on the NYU Dentistry website Click on the link. This will direct you to the Patient Access Portal home page. Enter your date of birth, first name and the e-mail address you have on file at NYU Dentistry. You will receive an email shortly with a link* to finish setting up your account.
At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.
The RWJBarnabas Health Patient Portal is a free service that provides convenient access to your personal health care information in a secure online environment. With the patient portal, you can view select test results, access your medical records and track your health information.
To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.

You can contact your local medical society who can help you contact the provider or doctor. The provider is not allowed to deny the patient and so you can request any document you require. The medical society or even state has records and registrations, so you can obtain the latest contact information.
6 yearsThe NYS Department of Health, however, requires medical doctors to retain records for any adult patients for 6 years. Minor patients are kept for 6 years and until one year after the minor reaches the age of 18 (whichever is longer). For hospitals, medical records must be kept for six years from the date of discharge.
