Location: The Medical Records Department is located on the lower level of Northwestern Medicine Central DuPage Hospital. Use the Women and Children's entrance and stop at the Guest Services desk for directions. Hours: Monday through Friday, 8:00 am - 4:30 pm. Phone: 630.933.6361.

Can medical records be faxed?

For your privacy and security, medical records cannot be faxed directly to patients. Please call to arrange for pick up or mail delivery of your records.
